Coaches and assistants:
Two Program admin forms, three livescan forms (one copy filled out by livescan site has to be returned to Westfield Soccer Club) the other two stays with the livescan operator. and photo.
Players transfering into Westfield Soccer form must fill out transfer/waiver form along with current processing fee.
Refund deadlines are as follows:
Fall season registration refund deadline is May 5th, an administrative fee may be imposed if having practiced, or played games/tournaments, otherwise no refund.
Spring season registration refund deadline is Dec 27, an administrative fee may be imposed if having practiced, or played games/tournaments, otherwise no refund.
Book keeping closes official financial books Monday after the end of season. Fall season books are closed the Monday after the last official end of season (Monday right before Thanksgiving). Spring season books are closed Monday before Memorial Day weekend.
